The 5 Star Hotel one-click demo import process installs the free version of Caldera Forms from the WordPress.org repository, but it does not import a pre-built Contact Form.
You can build your own Contact Form from scratch following the Caldera Forms documentation here.

Once your form is created, you must attach it to the Contact Us Page. To do this:
Navigate to: Admin Menu > Pages > All Pages > click "Edit" for the Contact Page > ensure you're editing with the Divi Builder > Edit the module labeled "Caldera Contact Form" > click the Caldera Forms button in the TinyMCE Editor (if using the Classic Editor) > select the Contact Form you've just created > click the "Insert Form" button > "Save & Exit" Module > Update/Save Page > view the Contact Us Page to ensure your Contact Form displays correctly.
